Manuscript, in unidentified hand, on parchment, containing Hippocrates' Aphorisms with Galen's commentary (translated by Constantinus Africanus), Galen's Tegni with Haly's commentary (translated by Gerardus Cremonensis), Hippocrates' Prognostics with Galen's commentary (translated by Gerardus Cremonensis), and Hippocrates' De regimine acutorum I-III with Galen's commentary (translated by Constantinus Africanus or Gerardus Cremonensis). Translations are from Arabic to Latin. Other Latin translations (without commentary) of Tegni, Prognostics, and De regimine acutorum in lower margins added late 13th Century.

- Heavily annotated in contemporary and later hands. Binder's stamp on front pastedown: Cirnuzevault. Bookplate of "Comtes Chandon de Briailles" with their MS number "MSS. 9"; likely owned by Comte Raoul (1850-1908) and Comtes Henri (1898-1937) and François (1892-1953). Sold in their sale, Rheims, February 17, 1955, lot 12. Puchased by Dr. and Mrs. John F. Fulton from the Internationaal Antiquariaat en Moderne Boekhandel (Menno Hertzberger), Amsterdam. Presented to Yale Medical Library in 1956 by John F. Fulton.
